Santol Seed Causing Intestinal Perforation Update – December 5, 2011

December 4 to 7, 2011 is the annual date for the Philippine College of Surgeons Annual Convention.

 

December 5, 2011, I attended the PCS Convention in EDSA Shangri-la.

 

While there, I met colleagues and former students, exchanging pleasantries and notes and other things.

 

Dr. Raul Desipeda was my contemporary in the Philippine General Hospital during our general residency years.  He is currently practicing in Lipa, Batangas.

 

Dr. Rolly Padre was a former student of mine during his general surgery residency years in PGH.  He is currently practicing in Sariaya, Quezon.

 

I did not bring up the topic of santol seed causing intestinal perforation when the three of us were together.  Rolly brought it up and was discussing it with Raul initially.  I just butted in and told them of my advocacy since 2001 warning the public on the danger of swallowing santol seeds.  Rolly said he had 3 cases whom he operated this year in Quezon.  Raul said he had a share of this kind of cases also.  All their patients were also in the older age group.

 

My insights when discussing the topic on santol seed causing intestinal perforation with Rolly and Raul:

 

The sharing of experience of these two surgeons practicing outside Metro Manila validated my 2001 alert call on the danger of santol seed swallowing.  The danger really exists in the Philippines.

 

I don’t have hard data on the incidence.  In 2001, I estimated it to be 200 cases per year, using 2 cases per year multiplied by an estimated number of government tertiary (regional, provincial, district, and city) hospitals in the Philippines (which I placed at about 100).  It will be hard to get an accurate national registry because utmost cooperation in reporting is needed.  At any rate, the sharing of experience of these two surgeons practicing outside Metro Manila tends to somehow support my rough estimation that considers occurrences of such cases in the provinces.
